To become a Hi-Tech PMR Client, the following information is required:
- Updated copy of the Business Information Form/Patriot Act (click here, please return 1st page)
- Copy of your article of formation for the business
- Copies of driver’s licenses for all owners listed on the article of formation and managers who handle business
- Copy of your business license
- Copy of your Federal EIN certificate and Sales Tax ID certificate
